Job summary

Cordova Wellness & Rehabilitation Center in Cordova, TN is seeking a compassionate Social Services Director to lead our Social Services Department. You’ll oversee program development, coordinate with residents and families, and ensure compliant, high-quality care.

The role requires a bachelor’s degree in social work or related field (or SW licensure) and at least one year of healthcare social work experience. On-site position with a collaborative team environment.

Qualifications

• A minimum of a bachelor’s degree in social work or another human services field such as sociology, gerontology, special education, rehabilitation counseling, or rehabilitation or be a licensed Social Worker.
• 1 year of supervised social work experience in healthcare.

Responsibilities

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.
• Complete assessments and participate in comprehensive care plans.
• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

Social Services Director Position Summary

As the Social Services Director at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:
