Spring View Nursing and Rehabilitation is seeking a Social Services Director for our skilled nursing facility. The role focuses on advocating for residents, coordinating with families, and leading psychosocial care planning to ensure dignity and comfort.

The Director collaborates with a multidisciplinary team to provide emotional support, coordinate resources, and ensure compliance with healthcare social services regulations in a resident-centered environment.

Qualifications

• Bachelor's degree in Social Work or related field.
• One year of supervised social work experience in a health care setting.
• Strong communication and interpersonal skills to interact with residents, families, and staff effectively.
• Empathetic and caring nature, with a genuine desire to improve the lives of our residents.
• Knowledge of relevant regulations and laws related to social services in a healthcare environment.

Responsibilities

• Assist residents with discharge planning and coordinate transitions to home or other care settings when necessary.

Social Services DirectorJob Duties

Qualifications of the Social Services Director:

• Minimum of a Bachelor's degree in Social Work or a bachelor's degree in a human services field, including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
• One year of supervised social work experience in a health care setting, working directly with individuals.
• Strong communication and interpersonal skills to interact with residents, families, and staff effectively.
• Empathetic and caring nature, with a genuine desire to improve the lives of our residents.
• Knowledge of relevant regulations and laws related to social services in a healthcare environment.
• Ability to work collaboratively with a diverse team and maintain a positive working environment.
