Job summary

Cedar View Rehabilitation and Healthcare Center is looking for a full-time Social Worker to join our passionate team. You will support residents by assessing needs and developing care plans that promote their well-being.

Your role includes evaluating psychosocial needs, addressing resident concerns, and coordinating care with interdisciplinary teams. We value education, and although a Bachelor’s degree is essential, a Master’s is preferred.

Qualifications

• Must be able to work full-time hours and maintain a flexible schedule.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting.
• Experience in a nursing home or rehabilitation center strongly preferred.

Full-time opportunity available

Salary range $25-$50 hr.

Responsibilities for Social Worker:

• Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications for Social Worker:

• Must be able to work full-time hours and maintain a flexible schedule.
• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.
