Job summary

Central Coast Hospice in San Luis Obispo is seeking a full-time Scheduler to join our Home Health team. The Scheduler will coordinate and schedule all team member care/visits and assist patients to ensure coordinated delivery of services.

The role requires strong communication, organization, and multitasking skills. Work hours are 8 AM–5 PM, Monday–Friday with weekend rotation; benefits include medical, dental & vision, 401(k), and more.

Qualifications

• Must have a license as an RN or LVN.
• Healthcare setting experience preferred.
• Excellent written & verbal communication skills.
• Ability to multitask with professionalism.
• Proficient in Outlook and Word; typing 40 wpm.

Responsibilities

• Coordinate and schedule all team member care/visits.
• Answer multiple phone lines for a variety of disciplines with accuracy and efficiency.
• Assist patients to ensure care is planned, coordinated and delivered appropriately.
• Re-prioritize workload as interruptions occur.
