Job summary

Angels Care Home Health is seeking an LPN Scheduler to manage scheduling of nursing visits using the Home Care Home Base system. The role collaborates with management and the interdisciplinary team to coordinate patient care and maintain accurate schedules.

Qualifications

• Minimum 1 year of home health experience.
• Excellent computer skills required.
• Excellent interpersonal and organizational skills.
• Basic knowledge of scanner/fax/copier equipment.
• Phone etiquette skills required.
• Typing ability 40-50 WPM.
• Knowledge of Medicare regulatory guidelines.

Responsibilities

• Schedule and appropriately document patient schedules in a timely manner.
• Participate in coordinating care with management and patient interdisciplinary team.
• Provide effective communication to patients, staff members, other health care professionals and referral sources.
• Respond to agency and patient needs in a professional and creative manner.
• Demonstrate commitment, professional growth and competency.
• Promote Agency philosophy and administrative policies to ensure quality of care.
