Job summary

Central Coast Home Health and Hospice in San Luis Obispo County is seeking a full-time Hospice RN to join our locally owned team. The RN will plan, organize, and direct hospice care in patients' homes, collaborating with an interdisciplinary team to honor patient and family choices and improve quality of life for those at end of life.

Schedule is Monday through Friday, 8 AM–5 PM with a 4- or 5-day option and rotating weekends; no night call.

Qualifications

• Current California RN license.
• CPR certified (American Heart Association).
• Minimum of one year of professional nursing experience.
• Reliable vehicle to drive for work and valid driver's license with clean driving record.
• Maintain auto insurance coverage.
• Home Health, Hospice, OR and/or ER experience preferred but not required.
• Understands hospice philosophy and issues of death and dying.

Responsibilities

• Plan, organize and direct hospice care in patients' homes and communities.
• Collaborate with an interdisciplinary team to meet patient and family needs.
• Promote patient and family choice and quality of life for those at end of life.
• Participate in care coordination and continuity of services.

San Luis Obispo -Coast (Los Osos -Cambria)

North County (Atascadero, Templeton, Paso Robles)

The nurse plans, organizes and directs hospice care while building community resources to plan and direct services to meet the needs of the patient and their families within their homes and communities. Collaborating with an interdisciplinary team to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients.

Work Schedule: regular work schedule is Monday-Friday, 8 AM- 5 PM with a choice between a 4 or 5 day work week, with rotating weekends (8AM-5PM).No night call!

Job Requirements:

• Current California RN license
• CPR certified (America Heart Association)
