Job summary

OPARC seeks a Case Manager (Licensed Programs) to coordinate person-centered plans and ISP development for participants in its licensed programs. You will manage a caseload of about 40, document meetings, and ensure records meet state and regional standards.

Responsibilities include developing PCP/ISP with participants and care teams, providing floor coverage as needed, and coordinating with IDT members while upholding safety and regulatory compliance.

Qualifications

• Must be at least 21 years of age.
• Associate’s degree in social work, Human Services, Business Administration, or related field, or minimum of one (1) year managerial experience.
• Knowledge of Title 17, Title 22, DDS, Regional Center and FLSA regulations.
• Demonstrates basic competency in behavioral support strategies and the implementation of person-centered intervention techniques.
• Organizational, communication, and leadership skills.

Responsibilities

• Coordinate and document annual, semi-annual, and quarterly meetings with participants and support team members to review progress on objectives.
• Maintain participant records in accordance with DDS, CCL, and Regional Center standards.
• Develop Person Centered Plans (PCP) and One-Page Descriptions, and use PCP to guide ISP development.
• Provide direct floor coverage and ratio support as needed to ensure participant safety.
• Facilitate Interdisciplinary Team (IDT) meetings to assess changes and coordinate services.
• Monitor participant health, medical, and behavioral concerns via tracking forms.
• Act as liaison to care providers, staff, and Regional Center Service Coordinators.
• Complete exit paperwork for participants and conduct a 6-month follow-up.
