Job summary

OPARC in Montclair, CA is hiring a Case Manager for Creative Arts and Explore programs. The role manages a participant caseload, develops Person-Centered Plans and ISPs, and coordinates assessments and meetings to monitor progress.

You will collaborate with families, care providers, and regional centers to ensure compliant, person-centered service delivery. Responsibilities include supervising activities, documenting outcomes, and advocating for participant goals while maintaining CARF and DDS

Qualifications

• Must be at least 21 years of age.
• Associate’s degree in Social Work, Human Services, Business Administration, or related field, or 1 year managerial experience.
• Knowledge of Title 17, DDS, Regional Center, and FLSA regulations.
• Organizational, communication, and leadership skills.
• Basic oral and written communication skills.
• Working knowledge of Microsoft Office and ability to learn internal systems.
• Ability to pass a DOJ background check and fingerprint clearance, TB test and pre-employment physical, and immunizations as applicable.
• Valid California driver’s license, proof of insurance, clear driving record, and two years of driving experience.
• Willingness to work in outdoor environments and varied settings.
• CPR, First Aid, and Crisis Intervention (CPI) certificates, or ability to obtain on hire.

Responsibilities

• Coordinate participant caseload management and develop Person-Centered Plans (PCP) and ISPs.
• Lead annual, semi-annual, and quarterly meetings with participants and the support team.
• Document progress, review notes, and ensure compliance with DDS/CARF standards.
• Provide advocacy, resource coordination, and barrier removal support.
• Assist with intake, eligibility verification, and employment documentation (I-9, W-4).
• Facilitate IDT meetings and coordinate services across teams.
• Support participants in daily activities and community-based outings as needed.
• Maintain participant records in accordance with policies and required records.
