Job summary

OPARC is seeking a Case Manager for Licensed Programs in San Bernardino, CA. The role manages a caseload of about 40 participants, coordinates services, and maintains compliant records across DDS, CCL, and Regional Center standards.

Strong organizational and leadership skills are essential, with a focus on person-centered planning and advocacy. Responsibilities include developing PCPs/ISPs, leading IDT meetings, and ensuring quality service delivery.

Qualifications

• Must be at least 21 years of age.
• Associate’s degree in social work, Human Services, Business Administration, or related field.
• Knowledge of Title 17, Title 22, DDS, Regional Center and FLSA regulations.
• Must possess a valid CA driver’s license with a clean driving record and two years of driving experience.

Responsibilities

• Manage a caseload of approximately 40 participants with regular progress reviews.
• Coordinate and document annual, semi-annual, and quarterly meetings to review objectives.
• Maintain participant records in accordance with DDS, CCL, and Regional Center standards.
• Develop Person Centered Plans (PCP) and Individual Service Plans (ISP) with participants and staff.
• Provide direct floor coverage and support during staff absences to ensure participant safety and continuity of operations.
• Lead Interdisciplinary Team (IDT) meetings and coordinate services.
