Zusammenfassung

Affinity Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN/LVN) to join our home hospice team on a per diem basis in California. You will provide direct patient care, support care plans, educate families, and help patients heal and remain safe in their homes.

This role emphasizes teamwork, empathy, and high-quality service. A CA LPN/LVN license, CPR, and reliable vehicle are required, plus 3+ years of nursing experience and a willingness to travel to patient homes.

Qualifikationen

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Aufgaben

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
