Job summary

Affinity Hospice Care Inc. seeks a dedicated Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) to provide compassionate home hospice care per diem in California, traveling to patient homes.

You will implement care plans, educate families, document accurately, and collaborate with RN, OT, PT, and HHA—part of the care team. Per visit rate is $50–$60, with a flexible schedule and potential to convert to full-time based on performance, plus mileage reimbursement.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and ability to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
