Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse to provide compassionate home hospice care, helping patients remain in the comfort of their own homes.

You will implement individualized care plans, educate families, and collaborate with an interdisciplinary team. CA LPN/LVN license, CPR, and ability to travel to patient homes are required; per diem schedule with potential for full-time conversion.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team – including OT, PT, HHA, and other disciplines – and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
