Job summary

Affinity Hospice Care is seeking an experienced Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in California for per diem home health visits. You will provide direct patient care, follow care plans, educate families, and document care accurately.

We value teamwork, compassion, and patient-centered service, with a flexible per diem schedule and the potential to convert to full-time based on performance. A valid CA license and CPR certification are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license and reliable insured vehicle with ability to travel to patient homes.
• Strong communication and collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
