Affinity Hospice Care Inc. seeks a per diem Licensed Practical/Vocational Nurse (LPN/LVN) in California to provide direct patient care in home hospice settings.

You will work under RN guidance to support patient safety, comfort, and well-being in the patient’s home. You must hold an active California LPN/LVN license, have 3+ years of nursing experience (home health preferred), CPR, and a valid driver’s license to travel to patient homes.

Responsabilités

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $50 - $60 per visit.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services

Requirements

• Graduate of an accredited practical or vocational nursing program
• Active LPN or LVN license in California
• 3+ years of nursing experience; home health or med/surg background preferred
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication skills and a collaborative, patient-first mindset
