Job summary

Affinity Hospice Care Inc. in California is seeking an experienced Licensed Practical Nurse (LPN) to deliver compassionate, in-home hospice care. You will work under RN supervision and support patients and families in making informed decisions.

This per-diem position emphasizes patient safety, accurate documentation, and collaboration with the care team. A valid CA LPN license, CPR certification, and reliable transportation are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active California LPN/LVN license.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
