Job summary

Affinity Hospice Care Inc. seeks a Licensed Practical/Vocational Nurse to provide hands-on home hospice care per diem in California. You will follow care plans, educate families, document care, and collaborate with a multi-disciplinary team to support patients in their homes.

Requirements include an accredited LPN/LVN program, CA license, 3+ years nursing experience (home health preferred), CPR (AHA), valid driver's license, and strong communication. Travel to patient homes is expected.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license with a reliable insured vehicle and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team—including OT, PT, HHA, and other disciplines—and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
