Job summary

Affinity Hospice Care Inc. is seeking a dedicated Licensed Practical/Vocational Nurse to deliver compassionate home-based hospice care per diem in California. You will work with a supportive care team to help patients heal, stay safe, and thrive in familiar surroundings.

As an LPN/LVN, you will follow individualized plans, educate families, document care, and travel to patient homes with a reliable vehicle. This role emphasizes empathy, teamwork, and patient-first care.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team – including OT, PT, HHA, and other disciplines – and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
