Job summary

Premier Home Health Providers Inc. is seeking a per diem Licensed Practical/Vocational Nurse to provide direct patient care in patients' homes across California.

You will implement care plans, educate families, and collaborate with an interdisciplinary team to ensure safe, quality recovery. The role requires a California LPN/LVN license, CPR certification, and at least 3 years of nursing experience, preferably in home health or med/surg.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active California LPN/LVN license.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnoses.
• Document care accurately and in a timely manner.
• Collaborate with the care team—including OT, PT, HHA, and other disciplines—communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $45 - $55 per visit.
