Job summary

Premier Home Health Providers Inc. is hiring a Licensed Practical/Vocational Nurse for per diem home health visits in California. You will deliver direct patient care in patients' homes, follow RN plans, educate families, and document care accurately.

Travel to patient homes with a reliable vehicle is required. The role offers flexible scheduling, opportunities to convert to full-time based on performance, and benefits like mileage reimbursement and paid leave.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable vehicle for travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team including OT, PT, HHA, and other disciplines.
• Use supplies safely and adhere to aseptic technique.
