Job summary

Premier Home Health Providers Inc. is seeking a per diem Licensed Practical/Vocational Nurse in California to provide direct in-home patient care and education. You will follow care plans, document progress, and collaborate with a multidisciplinary team to ensure safe, high-quality home health services.

The role emphasizes flexible scheduling, potential conversion to full-time based on performance, and mileage reimbursement for patient visits, with CPR/AHA certification required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
