Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for a per diem role in California to support patient care at home. You will follow individualized care plans and provide direct nursing services in patients’ homes.

The position requires a CA license, 3+ years of nursing experience (home health or med/surg preferred), and CPR certification. A reliable vehicle and the ability to travel to patient homes are essential, with a flexible schedule and Per Diem

Qualifications

• Must hold a California LPN/LVN license and active status.
• Minimum 3+ years of nursing experience; home health or med/surg preferred.
• Current CPR certification (AHA).
• Must have a valid driver’s license and a reliable insured vehicle with ability to travel to patient homes.
• Strong communication and collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team (OT, PT, HHA, and others) and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
