Best Home Health Providers Inc. in Santa Rosa, CA, provides compassionate home health care to patients across the Bay Area.

The per diem LPN/LVN role involves direct patient care, adherence to care plans, and education for patients and families, with a focus on safety and dignity in a home setting. The position requires an active CA LPN/LVN license, CPR certification, and 3+ years of nursing experience, preferably in home health or med/surg, plus strong communication and teamwork skills.

Qualifications

• Must be a licensed practical/vocational nurse (LPN/LVN).
• Active California LPN/LVN license and CPR certification.
• 3+ years of nursing experience; home health or med/surg preferred.
• Excellent communication and teamwork skills.

Responsibilities

• Provide direct patient care in accordance with state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnoses.
• Document care accurately and in a timely manner.
• Coordinate hospice services with other disciplines and relay needs to RN/Clinical Supervisor.
• Use supplies safely and adhere to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $50 - $60 per visit.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Participates in coordination of hospice services — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.
