Job summary

Affinity Hospice Care Inc. is seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide direct patient care in patients’ homes, following individualized care plans set by the RN. The role emphasizes compassionate, patient-first care in a home setting, with emphasis on safety and clear communication.

The position is per diem with a pay rate of $50–$60 per visit. Travel to patient homes is required, and candidates must have an active California LPN/LVN license and CPR certification.

Qualifications

• 3+ years of nursing experience; home health or med/surg background preferred.
• Active LPN/LVN license in California.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care per the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely with aseptic technique.
• Perform other duties as required for patient services.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.
