Job summary

Best Home Health Providers Inc. is seeking a qualified LPN/LVN to deliver high-quality home health care across California. This per diem role focuses on patient safety, comfort, and recovery in the home setting, with opportunities to travel to patient homes.

Under the supervision of an RN, you will implement care plans, educate patients and families, document care accurately, collaborate with the care team, and use supplies safely. A CA license, CPR, and a reliable vehicle are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services

Please note that this role is per diem with a rate of $45 - $55 per visit.
