Job summary

Best Home Health Providers, Inc. is seeking a Licensed Practical/Vocational Nurse to provide in-home patient care across the Bay Area and beyond on a per diem basis.

You will implement individualized care plans, educate patients and families, and document outcomes while collaborating with a multi-disciplinary team. Requirements include CA LPN/LVN license, 3+ years of nursing experience (home health preferred), CPR (AHA), and a valid driver’s license with reliable transportation.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services
