Job summary

Best Home Health Providers, Inc. in California is seeking a licensed practical/vocational nurse (LPN/LVN) to deliver direct patient care in home health settings. You will follow RN-drafted care plans, educate families, and document care accurately.

This per diem role offers a flexible schedule with opportunities to convert to full-time based on performance, and includes mileage reimbursement, paid sick days, bereavement leave, and ongoing skills training.

Qualifications

• Active LPN/LVN license in California.
• 3+ years nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $45 - $55 per visit.

Requirements

• Graduate of an accredited practical or vocational nursing program
• Active LPN or LVN license in California
• 3+ years of nursing experience; home health or med/surg background preferred
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication skills and a collaborative, patient-first mindset
