Job summary

Best Home Health Providers, Inc. is seeking a licensed practical nurse (LPN/LVN) in California to deliver high-quality home-based care under RN supervision. This per diem position offers flexible scheduling and direct patient interaction in patients' homes.

The ideal candidate has California LPN/LVN licensure, CPR certification, and 3+ years of nursing experience, preferably in home health or med/surg settings. Valid driver's license and reliable vehicle are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN/LVN license in California.
• 3+ years nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license and insured vehicle for travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team including OT, PT, HHA, and others; communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely and adhere to aseptic technique.

Please note that this role is per diem with a rate of $45 - $55 per visit.
