Job summary

Best Home Health Providers, Inc. is seeking a Licensed Practical Nurse (LPN/LVN) for per diem home health visits in California.

You will provide direct patient care, follow individualized plans, and educate families to support recovery at home. Qualified candidates have 3+ years of nursing experience (home health preferred), active CA LPN/LVN license, CPR certification, and reliable transportation to reach patient homes.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN/LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services

Please note that this role is per diem with a rate of $45 - $55 per visit.
