Job summary

Best Home Health Providers, Inc. is seeking a licensed practical/vocational nurse (LPN/LVN) for per diem home health visits in the San Jose area.

You will provide direct patient care, implement care plans, educate families, and collaborate with the care team to ensure safe, quality recovery at home. Requirements include CA LPN/LVN license, 3+ years nursing experience (home health or med/surg preferred), CPR, valid driver's license, and reliable vehicle.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active California LPN/LVN license.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with state Nurse Practice Act
• Implement and follow individualized care plans established by the RN
• Educate patients and their families on self-care techniques and diagnosis-related information
• Document care accurately and in a timely manner
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor
• Use supplies and equipment safely, adhering to aseptic technique
• Perform other duties as required for patient services

Please note that this role is per diem with a rate of $45 - $55 per visit.
