Job summary

Premier Home Health Providers Inc. seeks a licensed practical/vocational nurse (LPN/LVN) in California to provide compassionate home health care on a per-diem basis. You will deliver direct patient care, implement care plans, educate families, and document all activities in a home setting.

Trigging strong clinical skills with teamwork, you will travel to patient homes, maintain CPR certification, and collaborate with RNs and other clinicians to ensure safe, high-quality care.

Qualifications

• Licensed Practical/Vocational Nurse (LPN/LVN) licensed in California.
• Active CPR certification (AHA).
• Excellent communication and collaborative skills.
• Willingness to travel to patient homes and work independently.

Responsibilities

• Provide direct patient care per state Nurse Practice Act.
• Follow RN-established care plans and document care.
• Educate patients and families on self-care and diagnoses.
• Coordinate with care team and relay patient needs to RN/Clinical Supervisor.

Please note that this role is per diem with a rate of $45 - $55 per visit.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.
