Job summary

Best Home Health Providers, Inc. in California is seeking a dedicated Licensed Practical Nurse (LPN)/LVN to join our home health team on a per diem basis.

You will provide direct patient care, follow care plans, and educate patients and families to support recovery at home. Responsibilities include delivering care per state standards, documenting accurately, and collaborating with RNs and the care team to ensure safe, high-quality service across Bay Area communities.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active California LPN/LVN license.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team—including OT, PT, HHA, and other disciplines—and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $45 - $55 per visit.
