Job summary

Caring Hand Home Health Solutions, Inc. is seeking a per diem LPN/LVN to provide direct patient care in home settings across California.

You will implement established care plans, educate families, and collaborate with RNs and the care team to support patient recovery. The role requires a CA LPN/LVN license, 3+ years of nursing experience (home health or med/surg preferred), CPR certification, and ability to travel.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable vehicle for travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $45 - $55 per visit.
