Job summary

Interim HealthCare Inc. in Morristown, TN is seeking a Private Duty LPN or RN for full-time night shifts, Monday through Thursday 7pm to 7am, with weekends off.

Nurses are valued, supported by management, and empowered to advocate for patients while delivering high-quality home health care.As part of a nationwide network, you’ll work with RNs, CNAs and aides, maintain vitals, assessments, and care plans, educate families, and ensure a safe transition from hospital to home.

Qualifications

• Must be TN-licensed LPN or RN with active CPR certification.
• knowledge of state and federal home health regulations.
• Strong clinical skills and compassionate patient care.
• Excellent communication and teamwork abilities.

Responsibilities

• Provide home-based care to patients under RN supervision.
• Collaborate with physicians, RNs, CNAs and aides to deliver quality care.
• Take patient vitals, perform assessments, document progress, report changes to RN supervisor.
• Assist RN in carrying out patient plans of care and teaching self-care.
• Educate family on patient plans, medications and home safety.
• Coordinate care and discharge planning with interdisciplinary team.

As a Home Health Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Nurses:

• Graduate of an accredited practical nursing program and active RN OR LPN license in TN
• CPR certification
