Job summary

Interim HealthCare is seeking a Private Duty LPN or RN for full-time 36 hours per week in Bristol, TN. Shifts are 7pm-7am, Wednesday through Friday, with no weekend work and 1:1 patient care. Benefits begin after 30 days and weekly pay is offered.

You’ll be part of a national network that values nurses and advocates for patient-focused care. Responsibilities include monitoring vitals, performing assessments, and coordinating with RNs and other care team members to execute patient plans of care.

Qualifications

• Graduate of an accredited practical nursing program and TN RN or LPN license.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Excellent nursing skills and compassionate patient care.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN.
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care.
• Take patient vitals, perform assessments, document progress and report changes to RN supervisor.
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques.
• Educate family on the patient’s plan of care, medication and home safety.
• Collaborate with interdisciplinary team to coordinate patient care and discharge planning.
