Job summary

Interim HealthCare is seeking a Private Duty LPN or RN for full-time home health nursing in Johnson City, TN. Schedule: Monday, Tuesday, Wednesday and Friday 6:30am–4:30pm with weekends off, focusing on 1:1 patient care. You will work with a RN and collaborate to deliver quality care.

You will monitor patient status, document progress, educate families on the care plan and safety, and advocate for patients as part of a dedicated team. Benefits after 30 days, plus increased pay for this case.

Qualifications

• Graduate of an accredited practical nursing program with TN license (LPN or RN).
• CPR certification.
• Knowledge of state and federal home health regulations.
• Strong communication and compassionate patient care.

Responsibilities

• Provide home-based care to patients in Johnson City under RN supervision.
• Take patient vitals, perform assessments and document progress.
• Educate family on plan of care, medications and home safety.

As a Home Health Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Nurses:

• Graduate of an accredited practical nursing program and active RN OR LPN license in TN
