Job summary

A leading home health care provider is seeking a Private Duty Licensed Practical Nurse (LPN) for flexible, part-time shifts in Tri Cities, TN. Candidates must be graduates of accredited nursing programs with an active LPN license in TN. Responsibilities include providing home-based care, collaborating with a team of health professionals, and educating patients and families on care plans. Qualifications

• CPR certification required.

Responsibilities

• Provide home-based care to patients under the supervision of an RN.
• Take patient vitals, do assessments and document progress.
• Assist RN in carrying out patient’s plan of care.
• Assist RN with plan of care and patient self-care education.
• Educate family on care plan, medications, and home safety.
• Coordinate care and discharge planning with the interdisciplinary team.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
