Job summary

Interim HealthCare Inc. is looking for a dedicated Home Health Nurse in Blountville, TN, to provide one-on-one patient care. This full-time night-shift role (7p-7a) offers benefits after 30 days, including weekly pay and autonomy in care delivery.

As a Home Health Nurse, you'll collaborate with a healthcare team and educate families on care plans. Must hold a TN nursing license and CPR certification. Enjoy a supportive work culture and a meaningful role advocating for patients.

Qualifications

• Must have an active RN or LPN license in Tennessee.
• Graduate of an accredited practical nursing program.
• CPR certification required.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based care to patients under RN supervision.
• Collaborate with a healthcare team to deliver quality patient care.
• Take patient vitals, document progress, and report changes.
• Assist RN in implementing patient care plans.
• Educate family on care plans and medication.

As a Home Health Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Nurses:

• Graduate of an accredited practical nursing program and active RN OR LPN license in TN
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing, strong communication skills and compassion
