Overview

To provide practical nursing services under the supervision of a Registered Nurse.

Responsibilities

• Accept assignments of clients from the R.N. Case Manager.
• Provide nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
• Perform treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
• Observe signs and symptoms and report them to the R.N. Case Manager.
• Note changes in the physical or emotional status of clients.
• Promote client and caregiver independence in self-care.
• Utilize electronic documentation of client care; comply with Agency policies regarding the timeliness of submission and synchronization of data.
• Follow Agency policies regarding universal precautions and safety guidelines.
• Fulfill other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

Qualifications
