Job summary

CareRing Health is seeking a Licensed Practical Nurse to provide in-home nursing care under the supervision of an RN Case Manager. You will perform physician-ordered treatments, assess client status, and document care electronically in a home health setting.

The role emphasizes patient independence, clear communication, and adherence to safety policies. A Virginia LPN license and CPR certification are required, with at least one year of nursing experience.

Qualifications

Responsibilities

• Provides nursing care under RN Case Manager supervision using home assessments, nursing diagnoses and physician-approved care plans.

• Notes changes in physical or emotional status of clients and promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care and adheres to agency policies regarding timeliness and data synchronization.

What You Will Do

Accepts assignments of clients from the R.N. Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plansa. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.

b. Observes signs and symptoms and reports them to the R.N. Case Manager.

c. Notes changes in physical or emotional status of clients.

d. Promotes client and caregiver independence in self-care

Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of dataFollows Agency policies regarding universal precautions, and safety guidelinesFulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
