HomeRecovery HomeAid is seeking a Licensed Practical Nurse to provide nursing services under the supervision of a Registered Nurse in a home-based setting. The role emphasizes patient- and caregiver-centered care, with a focus on independence and adherence to physician-approved care plans.

Responsibilities include assessing client needs, observing and reporting status changes, and maintaining accurate electronic documentation in line with agency policies.

Qualifications

• Holds a current Licensed Practical Nurse license in Virginia.
• Minimum of one year of nursing experience.
• CPR certification currently valid.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision.
• Able to manage multiple projects and changing priorities.
• Able to employ critical thinking skills.

Responsibilities

• Accepts assignments of clients from the R.N. Case Manager and provides nursing care per the home assessment, nursing diagnoses, and physician-approved care plans.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
• Notes changes in physical or emotional status of clients.
• Promotes client and caregiver independence in self-care.
• Utilizes electronic documentation of client care; complies with agency policies regarding timely submission of documentation and synchronization of data.
• Follows agency policies regarding universal precautions and safety guidelines; fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

What You Will Do

Accepts assignments of clients from the R.N. Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plansa. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
