Job summary

Hebrew SeniorLife in Boston, MA is seeking a dedicated Licensed Practical Nurse (LPN) to collaborate with RNs in delivering high-quality care. You will participate in the nursing process, ensure safety and dignity, and support culture change initiatives across units.

As a key team member, you will mentor patient care associates, participate in governance, and uphold HIPAA requirements while maintaining strong communication with the multidisciplinary team.

Qualifications

• A graduate of an approved school of nursing is required.
• Current Massachusetts license as an LPN in good standing is required.
• Previous experience preferred.
• Understand the philosophy and support the model of care at HSL.

Responsibilities

• Ensures all care delivered is within the scope and established standards of safe professional nursing practice with core care competencies.
• Collaborates with the multidisciplinary team (MD, NP, nurses, social worker, therapists, dietitian) to meet patient needs and monitor outcomes.
• Adheres to best practice recommendations and implements evidence-based interventions.
• Practices in a primary nursing model and maintains accountability for a set of assigned patients with RN oversight.
• Collaborates in care planning activities and supports autonomy and individual choice.
• Documents pertinent information in the electronic medical record and initiates necessary reports.
• Identifies resources to ensure safe and effective care; delegates and supervises care by nursing assistants.
• Demonstrates critical thinking and models professional behavior; participates in governance and process improvements.
• Acts as a resource to PCAs and responds to safety concerns; maintains HIPAA compliance.
• Performs unit activities and may assist with meals; may assume charge nurse duties with RN oversight.
