Job summary

Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, is seeking a Licensed Practical Nurse for night PRN shifts at a brand-new 40-bed inpatient rehabilitation facility. The role focuses on patient assessments, timely nursing interventions, and educating families throughout rehab stay and discharge.

The ideal candidate holds an Associate's Degree, active Wisconsin (or compact) LPN license, BLS within 30 days, and about one year of preferred experience in a fast-paced rehab setting.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively with the interdisciplinary team.
• Participates in CQPI.
• Performs timely documentation relating to medical necessity in the medical record.
• Maintains and revises Interdisciplinary Care Plans for each patient.
• Performs timely and accurate QI assessments.

How you'll contribute

A Nurse who excels in this role:

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What we are looking for:

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsinor compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
