Job summary

Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, invites you to join our brand-new 40-bed inpatient rehabilitation facility as a Licensed Practical Nurse on the Night shift (PRN). You will provide essential nursing care, assessments, and education to patients recovering from injuries and illnesses in a supportive, collaborative environment.

We seek a dedicated LPN with an Associate's degree, an active Wisconsin LPN license (or compact license), BLS certification within 30 days,

Qualifications

• Must have an active Wisconsin LPN license (or compact state).
• Associate's degree required.
• BLS certification within 30 days of hire.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively with other members of the interdisciplinary team.
• Participates in CQPI initiatives.
• Performs timely documentation related to medical necessity.
• Completes and revises the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

How you'll contribute

A Nurse who excels in this role:

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What we are looking for:

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsinor compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
