Rehabilitation Hospital of Oak Creek is seeking a Licensed Practical Nurse for night shifts to join our new, 40-bed inpatient rehabilitation facility. You will assess patients, plan and implement nursing interventions, and educate patients and families throughout the rehabilitation stay.

The role requires an Associate's degree, an active Wisconsin or compact LPN license, and BLS within 30 days of hire. Prior experience is preferred and you will collaborate with an interdisciplinary team to

• Minimum 1 year of prior nursing experience preferred.
• Associate's degree in nursing is required for LPN role.
• Active Wisconsin or compact LPN license is required and must be updated if relocating.

How you'll contributeA Nurse who excels in this role:

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What we are looking for:

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsinor compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
