Job summary

Licensed Practical Nurse | Rehab at Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, seeks a dedicated LPN for night shift. Join a new 40-bed inpatient rehab unit and contribute to high-quality patient care in a multidisciplinary team.

You will perform assessments, initiate nursing interventions, and educate patients and families throughout the rehab stay, ensuring safe discharge planning and continuity of care.

Qualifications

• Associate's Degree in Practical Nursing required.
• Active Wisconsin LPN license (or compact) required.
• BLS certification within 30 days of hire.
• One year of experience preferred.

Responsibilities

• Perform accurate patient assessments and identify needs.
• Initiate appropriate nursing interventions.
• Provide patient and family teaching during rehab stay and at discharge.
• Deliver care appropriate to patient condition and age.
• Collaborate with interdisciplinary team members.
• Participate in CQPI and continuous improvement activities.
• Document medical necessity timely and accurately.

Rehabilitation Hospital of Oak Creek is a brand-new 40 bed inpatient rehabilitation facility.We offer high-quality, compassionate care for a wide variety of patients in need. How you'll contribute

A Nurse who excels in this role:

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What we are looking for:

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsinor compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
