Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, is seeking an LPN to join nights on a PRN basis. You will perform assessments, implement nursing interventions, and provide patient and family teaching throughout the rehab stay.

This role requires an Associate's degree, an active LPN license (Wisconsin or compact), BLS within 30 days, and a mindset for teamwork within an interdisciplinary group. Prior rehab experience is preferred, with a commitment to compassionate care.

• BLS/CPR training preferred, with BLS within 30 days of hire.
• One year nursing experience preferred.

Rehabilitation Hospital of Oak Creek is a brand-new 40 bed inpatient rehabilitation facility.We offer high-quality, compassionate care for a wide variety of patients in need. Westrive to maximize the health, function, and quality of life of those we serve through comprehensive physical medicine and rehabilitation programs.

How you'll contribute

A Nurse who excels in this role:

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What we are looking for:

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsinor compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
