Job summary

Rehabilitation Hospital of Oak Creek in Wisconsin seeks a Licensed Practical Nurse to join our brand-new 40-bed inpatient rehabilitation facility. This full-time role offers day shifts and a chance to provide direct patient care within a collaborative interdisciplinary team.

You will perform assessments, initiate nursing interventions, educate patients and families, and help develop care plans while documenting accurately for medical records.

Qualifications

• Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision.
• Experience: One year previous experience preferred.
• Licenses: Active Wisconsin or compact state LPN license required; licensure updates to PSOR if relocating.

Rehabilitation Hospital of Oak Creek is a brand-new 40 bed inpatient rehabilitation facility. We offer high-quality, compassionate care for a wide variety of patients in need. We strive to maximize the health, function, and quality of life of those we serve through comprehensive physical medicine and rehabilitation programs.

How you'll contribute

A Nurse who excels in this role:

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What we are looking for

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsin or compact state LPN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
