Traditional Home Health Care is looking for a compassionate Licensed Practical Nurse to join our team providing high-quality in-home care for pediatric and adult patients in their own homes.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Shift:

Weekday and weekend shifts, full-time and part-time schedules available!

Rate:

$28-$32 per hour

Immediate Need

Mount Pocono, PA: Monday - Friday 8:00am-4:00pm G-tube experience needed

Cresco, PA:

Saturday & Sunday 7am-3pm / Monday - Friday overnight shifts available, large breed dog in the home

Tobyhanna, PA:

Monday - Friday 8am-8pm & Saturday & Sunday 9am-6pm
