Job summary

Elara Caring is seeking a Licensed Practical Nurse (LPN) to provide home health services in the Benton Harbor area and surrounding counties. This full-time role offers Monday–Friday daytime hours and requires travel between patient homes as assigned.

You will follow individualized care plans, monitor patient progress, document care activities, and collaborate with RNs and physicians to ensure safe, effective care. Strong communication and caregiving skills are essential.

Qualifications

• State license as LPN is required.
• Minimum 1 year of clinical care experience.
• Ability to lift and move patients up to 50 pounds; driver’s license or reliable transportation.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with RN Case Manager as patient needs change.
• Document medication regimens per agency guidelines.
• Communicate with physicians, patients, families and the care team for coordinated care.
• Help prevent avoidable hospitalizations and ER visits through proactive patient care.
