Elara Caring is seeking a Licensed Practical Nurse (LPN) to support Rockford, IL and surrounding areas in home health. The role requires current LPN licensure, a nursing degree, and at least 1 year of clinical experience.

You will follow care plans, monitor progress, and help patients and families with self-care techniques. Travel between patient homes is required; a valid driver’s license or reliable transportation is necessary.

Qualificações

• Current State License as a Licensed Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
